The Patient Centred Medical Home model of care puts the patient at the centre of their care, helping to ensure each patient has the care team they need. By 2030, it is expected that there will be a 25% increase in the number of people aged over 65.
As the population aged over 65 steadily increases the frequency of health services being used will also increase. There is also a growing demand for those under 65 who are currently living with chronic health conditions. Considering the changing needs of our population, our current system model is not sustainable to ensure patients get the right care, at the right time and in the right place.
The Patient Centred Medical Home model will help to address the change required in the system and underpins the work we do at COORDINARE.
