COORDINARE invited general practices to apply for funding in 2020 to implement a quality improvement activity in general practice to improve patient outcomes during the flu season.
Building on the work completed for the 2019 Winter Strategy, as well as recent hospitalisation data from both the Illawarra Shoalhaven Local Health District (ISLHD) and Southern NSW Local Health District (SNSWLHD), COORDINARE sought to support enhanced practice capacity and heightened quality patient care by promoting a team based care approach.
Practices also had the opportunity to use the Patient Activation Measure (PAM) tool at baseline and completion of the implementation of their model of care.
By submitting an online application, practices outlined the quality improvement they would like implemented in their practice. A panel reviewed the applications and practices who were successful received support and funding to develop and implement their quality improvement initiative.
