COORDINARE is inviting general practices to apply for funding in 2020 to implement a quality improvement activity in general practice that will improve patient outcomes during the flu season.
Building on the work completed for the 2019 Winter Strategy, as well as recent hospitalisation data from both the Illawarra Shoalhaven Local Health District (ISLHD) and Southern NSW Local Health District (SNSWLHD), COORDINARE seeks to support enhanced practice capacity and heightened quality patient care by promoting a team based care approach.
Practices will also have the opportunity to use the Patient Activation Measure (PAM) tool at baseline and completion of the implementation of their model of care.
Simply submit an online application, outlining the quality improvement you would like to implement in your practice. A panel will review the applications and practices who are successful will receive support and funding to develop and implement their quality improvement initiative.
