End of life is that period of time when a person is living with an advanced, progressive, life-limiting illness. One in five residents in South Eastern NSW is aged 65 years and older, and there is a higher than the state and national average rates for premature mortality within the region.
End of life care is a key identified priority area for COORDINARE. We want to ensure that all people in South Eastern NSW have an optimal end of life experience, building on the strengths of care delivery and ensuring care and services are equitable, accessible, coordinated and culturally appropriate as the person experiences end of life. Consumer and Carer Relief at End of Life Project – Palliative Care NSW
The project focuses on developing volunteer based support in the southern region, and will include improving the availability and capacity of volunteers to offer home based support to consumers and their carers at end of life. Visit the Palliative Care NSW website.

The aim of this project is to have all patients with a terminal condition or advancing age to have a clear management plan as they undertake the end of life journey.
Strengthening community based palliative and end of life care in Upper Lachlan Shire - The Health Care Centre - Round 1
The aim of the project is to improve current pathways for palliative and end of life care working closely with patients, their GPs and providers to ensure the patient receives multidisciplinary care that is stepped, timely and appropriate for the Upper Lachlan Shire region. Caring safely for palliative patients in the last days of life - Batehaven Pharmacy

A model to strengthen capacity and coordination of palliative and end-of-life care in Cooma residential aged care - Australian National University
The aim of the project is to strengthen integrated palliative and end of life care service capacity within Cooma residential aged care (RAC) via: strengthening the utilisation of advance care directives, enhancing primary care service models, and improving coordination and integration between providers of palliative and end of life care.
