About Lumos

Lumos is a new partnership initiative between COORDINARE, the Southern NSW and Illawarra Shoalhaven Local Health Districts and the NSW Ministry of Health (MoH). The Lumos program links de-identified patient records from general practices with NSW Health records, to map patient journeys across the NSW health system. Lumos generates new insights to improve health services and patient outcomes across the state. 

The benefits to your practice

Lumos provides new insights on how your patients are interacting with the health system. This unique information can be used to inform your practice operations.

Consenting practices receive a report approximately every 6 months, supported by COORDINARE. Reports include information about the services your patients access, such as emergency department presentations and hospital admissions.

This can support you to:

  • Understand the characteristics of your patient population in the context of your region and state.
  • Identify opportunities to inform your practice operations.
  • Understand the NSW health services your patients access outside of your practice.
  • Support your data-driven quality improvement activities.

Participation is free of charge and does not impact the operations of your practice.

Protecting privacy

The Lumos program is ethically approved and has rigorous governance structures with GP and PHN representation. State-of-the-art privacy and security measures are embedded throughout the program to minimise risk to patient and practice privacy.

This initiative commenced in SENSW in 2018 with five early adopting general practices: Bega Valley Medical Practice, Goulburn Health Hub Medical Centre, Gwynneville Medical Practice, Windang Beach Family Medical Practice and Woonona Medical Practice.
