Nurse-led clinics are an alternative model of care delivery where the nurse is the primary provider of care for the patient. In the general practice setting, nurse clinic models support a team-based approach to care delivery, and involve the general practitioner and other members of the practice team.

In 2017-18, the Patient Centred Medical Home Innovation Project (PCMH IP) supported practices to develop and implement their own ideas for service improvement. Through this project local practices documented their models of care, some of which have been identified for scale up across the region.

Building on this PCMH work, COORDINARE offered practices an opportunity to implement one of the nurse-led clinic models, with the aim of delivering enhanced care for patients at risk of poor health outcomes.
