The Lumos program links de-identified patient records from general practices with NSW Health records, to map patient journeys across the NSW health system. Participation is free of charge and provides insights on how your patients are interacting with the health system.
Consenting practices receive a report every 6 months with information about the services your patients access, such as emergency department presentations and hospital admissions. This can support you to:

The Lumos program is ethically approved and has rigorous governance structures with GP and PHN representation. State-of-the-art privacy and security measures are embedded throughout the program to minimise risk to patient and practice privacy.
This initiative commenced in South Eastern NSW in 2018 with five early adopting general practices: Bega Valley Medical Practice, Goulburn Health Hub Medical Centre, Gwynneville Medical Practice, Windang Beach Family Medical Practice and Woonona Medical Practice.
