What is Fast Forms?

Fast Forms is an electronic referral system designed to offer general practices increased operational efficiency, reducing reliance on traditional paper or PDF forms by directly and instantly transmitting data into the recipient’s nominated system via secure messaging technology.


General practices interested in using Fast Forms can follow the registration steps below.

    STEP 1

    Register for a Fast Forms account here. Minimum requirements:

    • current NASH certificate and HPI-O number
    • Best Practice (version Lava Edition or Indigo SP2) or Medical Director (versions 3.17 or 3.18)
    • Windows (i.e. currently no Apple products) 
    • FireFox, Microsoft Edge or Chrome browser.

    STEP 2

    Telstra Health will contact you to provide a Fast Forms welcome pack including installation instructions and resources.

    Installation of Fast Forms is required on each machine and may require support from your IT team for administrative access.
