Commissioned service partners
Appropriate end of life care optimises the quality of life for individuals, their families, and carers. It focuses on preventing / reducing a person's suffering, and promotes dignity in dying.
We partner with a range of organisations and stakeholders to build capacity across our communities. We commission services for people living with life-limiting illnesses, and which support end-of-life planning.
