MyMedicare Patient Registration Pathway for patient:

• Do you have a Medicare card or DVA card? 
  • If yes, you can register from 1 October 2023: at a practice that you have made two face-to-face visits in the past two years, OR 
  • through your Medicare online services (Medicare Online Account or the Express Plus Medicare mobile application). 
• Patients who register with a DVA Veteran Card will need to fill out a registration form at the practice as registration via Medicare online services can only be completed using a Medicare card. 
• Practices will then need to accept your nomination before you are registered.  

The MyMedicare manual registration form is available for general practices to download and provide to their patients.

Geographically phased rollout of new incentives for general practices to provide wraparound care for people with complex, chronic conditions who are frequent hospital users.

Aged care SIP will be replaced with SIP/PIP combination (70% to GP, 30% to practice) rewarding quality and continuity of care (health assessments, care plans, regular visits). Current $10k cap removed.

Hospital frequent attendees will attract incentives (funding amount to be confirmed) and reward payments for reduction in hospital attendance. This measure will be rolled out progressively, commencing with nine PHN regions that are yet to be advised. 

Chronic Disease Management items linked to a patient’s registration in MyMedicare from November 2024, to support continuity of care for people with chronic and complex conditions. Patients who are not registered in MyMedicare will still be able to receive chronic disease management items from their usual GP.

• Removal of item 723.
• Removal of additional steps (e.g. fax back).
• Rebalancing of items 721 and 732 (significant increase in item 732 and slight drop in item 721).
• In the future additional cohorts will be incentivised e.g. mental health.
