COORDINARE - South Eastern NSW PHN is part of a nation-wide group of Primary Health Networks (PHNs). Each PHN aims to improve health service efficiency and effectiveness as well as care coordination to ensure community members receive the right care in the right place, at the right time.

We aim to ensure the best possible health and community care system for the people of South Eastern NSW, through strong partnerships with our stakeholders and community.

We acknowledge the Traditional Custodians of the land on which we work and pay our respects to their Elders past, present and emerging.

We also recognise, respect and affirm the central role played in our work by people with lived experience, their families and/or carers.

We thank our stakeholders, partners and staff who agreed to appear in this video and acknowledge our work would not be possible without them.
